On February the 3rd it was announced that the Nano Ledger S was vulnerable to a man in the middle attack. The exact way this vulnerability is exploited can be found outlined in the following document.

Ledger have now released firmware update 1.4.1 which fixes the man in the middle vulnerability while also adding many other cool features.
Some of the other improvements include the number of apps which can be installed on the ledger has been raised to up to 18, a great improvement from the previous firmware’s limit of 4 applications installed.
You can visit the ledger website for a full change log and to download the new firmware. Remember to update your firmware ASAP and if you get stuck on the “update” screen uninstalling any ledger drivers and trying again should get you going again.
