One expatriate came to me on Tuesday morning. He was suffering from an on-and-off fever for the last 9-10 days. He already received some medicines, including an antibiotic from another clinic, but didn't get better. He indeed looked sick! So, I advised on some investigations. His Widal test (a test for typhoid fever) came back negative. However, the CBC test revealed that his total platelet count was very low. It was only 19,000 cells while the normal limit is over 150,000 cells! An interesting matter was that the lab technicians found malaria parasites while examining his blood under a microscope! I wanted to do the malaria test in the beginning, but he didn't give any recent travel history. Hence, I skipped! But, good thing that the lab technician could see the parasite in his blood! In the country from which he is, malaria is quite prevalent. He last visited his country around 8-9 months ago. It looks like he was carrying the parasite in his blood for a long time! We referred the patient to the higher authority. He will be treated in the Government hospital/clinic.
Another patient came. He was sweating and shivering due to the excessive pain in his right lower abdomen. The pain started the previous night. He vomited several times. It appeared to be a ureteric stone patient. However, as the pain was in the right lower abdomen, acute appendicitis was another differential diagnosis. With three different types of painkiller injections, the pain finally settled down after one hour. We then did a urine test, and the report came in favor of stones. I asked them to do a scanning test. They returned later with a scan report. It revealed both-sided kidney stones but didn't mention any ureteric stones that could cause such severe pain!
On Tuesday morning, I walked on the treadmill for half an hour. Later, after lunch, I wanted to walk again but couldn't. Rather, I had a long afternoon nap. I managed to make over 12k steps on the day!
At work, I consulted 20 patients in my chamber. Hence, it wasn't a busy day. In the visa medical section, I examined some people who came for the fitness test. I could study a bit, and I worked online from the office desktop as well!
