Hoy, 5 meses después, vuelvo para darle cierre a otra de mis rotaciones, en este caso: Pediatría.
El domingo, me dieron la nota final de la rotación. Si, hay profesores que califican durante el fin de semana (yo también creía que no existían). Después de un poco de estudio, cogí 5, en mis exámenes práctico y teórico final, cerrando con la máxima nota posible. Otra para el saco, diría mi mamá.
Se terminaron: 15 semanas, donde roté por más de 10 subespecialidades. Una nueva cada semana. La verdad, se me pasó, más rápido de lo que yo esperaba. Con esto, terminó mi cuarto año. Bienvenido sea quinto año.
Por cierto, son las primeras vacaciones de fin de año que no tengo que estudiar
Pediatría, es la especialidad más parecida a Medicina Interna que hay, esperaba que me gustara mucho más que las anteriores (Ginecología y Obstetricia y Cirugía) y terminar el año motivado.
Acompáñame para hacer un poco de balance, lo bueno y lo malo de mi rotación por Pediatría, vamos a ver que sale:
Cuando era niño y me enfermaba, recuerdo que los viejos de la familia decían: Dale la mitad de lo que le dan a los adultos.
El primer día de rotación, lo primero que nos dijeron fue: El niño no es un adulto pequeño. Esto signifca que: el niño tiene sus tratamientos y dosificaciones especiales. Aunque los remedios familiares, pudieron hacerme efecto, la verdad es que: pudieron haberme hecho más daño que bien. Así que, no cometas estos errores.
Durante la rotación, aprendimos las diferentes dosificaciones para cada edad y con ello los tratamientos, esto nos ayuda hasta en nuestra futura labor como padre.
Esta fue la rotación donde terminé, aquí hice mi práctico. Tuve la suerte de coincidir con el profesor: Fernando Domínguez Dieppa, una eminencia en Neonatología. De verdad, el mejor profesor que tuve en esta rotación, con un conocimiento de Pediatría y cultura general, como pocos que he visto. Me gustó, la forma que tiene de dirigirse a sus alumnos y como se ve que le gusta lo que hace. Un ejemplo a seguir apra muchos educadores.
Curiosamente, el fue el neonatólogo que me atendió cuando yo nací, en el hospital, del cual hoy soy estudiante. Más curioso aún, es que también fue el Neonatólogo, de mucho de mis compañeros. Imagínate el orgullo de ese señor.
La rotación se basaba: en las mañanas, las clases o las consultas y la tarde libre.
Si miras mi perfil, te darás cuenta, desde septiembre, aumentó muchísimo mi ritmo de publicación y es por tiempo libre que tenía.
Casi siempre, estaba en mi casa antes del mediodía.
Aunque esto puede llegar a ser malo, la verdad es que en mi caso, me da una autonomía que me gusta. Me he acostumbrado a hacer sesiones de estudio intensos, es decir, me siento unas 2-3 horas y lo hago todo, a veces tengo que hacer más de uno por la cantidad de conteido que tenemos que estudiar.

El resto del tiempo, lo dediqué o adquirir otras habilidades médicas, o las cosas que me gustan.
En esta rotación, el tiempo libre fue lo que más me gustó, en parte, me motivó cuando otros aspectos de la rotación no lo hacían.
Ya no hay mucho más que añadir, pasemos a la otra cara de la moneda
Para nadie es un secreto: la intensa crisis de salud que vive Cuba. De esta nadie escapa.
El tema más polémico. Para suerte o desgracia, en el momento que rotamos por el Hospital Pediátrico "Marfán", ubicado en el Vedado; estaba siendo renovando su cuerpo de guardia. Sumando a la escasez de medicamentos, las madres prefieren llevar a sus hijos a otros hospitales, o directamente ni llevarlos y automedicarlos en casa.
El resultado: llegaba a las guardias y a la hora nos daban la salida. Fue una total pérdida de tiempo, varias veces, hacíamos las guardias y no recibíamos ni un solo paciente. Es algo que el Ministerio debería valorar pero todos sabemos que eso no va a cambiar.
Cuando un profesor llegaba a impartir una clase, o llegabamos a una guardia, lo primero que preguntaban era: si alguno íbamos a ser Pediatra. En mi brigada (aula) coincide en que nadie quiere esta especialidad. El caso, es que al recibir un NO, por parte de los estudiantes,los profesores cambiaban mucho su actitud, disminuyendo la calidad de la docencia que impartían.
Es cierto, que los médicos, son seres humanos y sufren la misma crisis que todos los demás, pero se supone que estos médicos cobran por dar docencia y deberían mínimamente esforzarse. En parte, los entinedo; la situación en el país no es alentadora y el sector de la salud es uno de los más afectados y eso se nota en cada especialista, si yo estuviera en sus lugares tal vez haría lo mismo.
Este fue una de los aspectos que más me desmotivo de toda la rotación. Sumado al próximo ...
Un médico, necesita práctica para su formación. Los viejos médicos son los mejores médicos por la cantidad de pacientes que han visto. Siguiendo esta lógica, un estudiante, necesita de la sala para adquirir los conocimiento más rápido y en Pediatría es lo contrario, escasa sala.
Mi tutor, dice que: lo que haces una vez, más nunca se te olvida y es todo lo correcto, haciendo se aprende más que estudiando.
Haciendo memoria, de las 15 semanas, solo fui 3 semanas, a la sala. Si vi pacientes, en las otras rotaciones pero no igual que en la sala. Esto sumado al punto anterior, agrandaron mi desmotivación.
Desmotivación, que para mi asombro, no fue mía solamente, mis compañeros expresaron lo mismo el último día de clases.
Aunque esta rotación tuvo muchas cosas desmotivantes y adversas, me niego a verla como una rotación mala, o perdida.
Llegué a la conclusión, que ha sido la rotación que más aportó a mis conocimientos médicos (no por mis profesores, sino por el autoestudio que tuve que hacer) Me permitió refrescar contenidos, que son importantes como Internista, la similitud, entre especialidades, me permitió repasar los contenidos tanto para adultos como para menores.
En este tiempo, reafirmé que la decisión que tomé hace tiempo, es la correcta. Mi objetivo sigue siendo convertirme en Internista y cada día estoy más cerca.
El semestre pasado, me enseñó a sobreponerme a las adversidades. Pediatría me enseñó a resistir y tener paciencia. La rotación no me gustó, los profesores se encargaron de eso pero la verdad es que ya estoy más cerca de mi meta y eso es todo lo que importa. Seguir acumulando contenidos, habilidades y pasando el tiempo ha sido siempre el objetivo hasta sexto año y ya que poco. Eso es lo que me llevo, de estas 15 semana.

Today, 5 months later, I'm back to wrap up another one of my rotations, this time in Pediatrics.
On Sunday, I received the final grade for the rotation. Yes, there are professors who grade over the weekend (I didn't believe it either). After some studying, I scored a 5 on my practical and final theoretical exams, ending with the highest possible grade. Another one for the record, as my mom would say.
It's over: 15 weeks, rotating through more than 10 subspecialties. A new one every week. Honestly, it flew by faster than I expected. With this, my fourth year comes to an end. Welcome, fifth year.
By the way, these are the first holiday vacations where I don't have to study.
Pediatrics is the specialty most similar to Internal Medicine. I was hoping I would like it much more than the previous ones (Gynecology and Obstetrics, and Surgery) and finish the year motivated.
Join me as we take stock, the good and the bad of my Pediatrics rotation. Let's see what comes out:
When I was a child and got sick, I remember the older folks in the family saying, "Give him half of what adults take."
On the first day of rotation, the first thing we were told was, "The child is not a small adult." This means that children have their own treatments and special dosages. Although home remedies might have worked for me, the truth is that they could have done me more harm than good. So, don't make these mistakes.
During the rotation, we learned about different dosages for each age group and the corresponding treatments. This knowledge will even help us in our future roles as parents.
This was the rotation where I finished, and I did my practical here. I was fortunate to coincide with Professor Fernando Domínguez Dieppa, a renowned expert in Neonatology. Truly, he was the best professor I had during this rotation, with a deep understanding of Pediatrics and general knowledge that few possess. I liked the way he interacted with his students, and it's evident that he loves what he does. He sets an example for many educators to follow.
Interestingly, he was the neonatologist who attended to me when I was born at the hospital where I am now a student. Even more curious, he was also the neonatologist for many of my classmates. Imagine the pride that man must feel.
The rotation consisted of classes or consultations in the mornings and free time in the afternoons.
If you look at my profile, you'll notice that my publishing frequency increased significantly since September, thanks to the free time I had.
I was usually back home before noon.
Although this can be seen as a downside, the truth is that in my case, it gives me a sense of autonomy that I enjoy. I've gotten used to intense study sessions, where I sit down for 2-3 hours and cover everything. Sometimes I have to do more than one session because of the amount of content we have to study.

The rest of the time, I dedicated to acquiring other medical skills or doing things I enjoy.
In this rotation, the free time was what I liked the most. It motivated me when other aspects of the rotation didn't.
There's not much more to add. Let's move on to the other side of the coin.
It is no secret to anyone: the intense healthcare crisis that Cuba is experiencing. Nobody can escape from it.
This is the most controversial topic. Unfortunately, during our rotation at the "Marfán" Pediatric Hospital located in Vedado, the emergency department was being renovated. Adding to the shortage of medications, mothers preferred to take their children to other hospitals, or worse, not take them anywhere and self-medicate them at home.
The result: we would arrive for our shifts and be dismissed after an hour. It was a complete waste of time. Several times, we would do our shifts and not receive a single patient. This is something the Ministry should consider, but we all know that it won't change.
When a professor came to give a lecture or when we arrived for a shift, the first thing they asked was if any of us were planning to become a pediatrician. In my group, it was unanimous that no one wanted to pursue this specialization. When the professors heard a "NO" from the students, their attitude changed significantly, and the quality of their teaching decreased.
It is true that doctors are human beings and they suffer from the same crisis as everyone else. But they are paid to teach, and they should make at least some effort. I understand them to some extent; the situation in the country is not encouraging, and the healthcare sector is one of the most affected. This is evident in every specialist. If I were in their shoes, I might do the same.
This was one of the aspects that demotivated me the most during the entire rotation. And it was compounded by the following...
A doctor needs practice for their training. Experienced doctors become the best doctors because of the number of patients they have seen. Following this logic, a student needs exposure to patients in order to acquire knowledge quickly. However, in Pediatrics, it is quite the opposite. There was a scarcity of hands-on experience.
My tutor says that what you do once, you never forget, and he is absolutely right. You learn more by doing than by studying.
Looking back, out of the 15 weeks, I only spent 3 weeks in the clinical setting. While I did see patients during the other rotations, it was not the same as in the clinical setting. This, combined with the previous point, deepened my demotivation.
To my surprise, my lack of motivation was not unique. On the last day of classes, my classmates expressed the same sentiment.
Although this rotation had many demotivating and adverse aspects, I refuse to see it as a bad or wasted rotation.
I came to the conclusion that it has been the rotation that contributed the most to my medical knowledge, not because of my professors, but because of the self-study I had to do. It allowed me to review important content as an internist. The similarities between specialties allowed me to review content for both adults and children.
During this time, I reaffirmed that the decision I made a while ago was the right one. My goal is still to become an internist, and every day I am getting closer.
Last semester taught me how to overcome adversity. Pediatrics taught me to persevere and have patience. I didn't enjoy the rotation, the professors made sure of that, but the truth is that I am closer to my goal, and that's all that matters. Continuing to accumulate knowledge, skills, and time has always been the objective until the sixth year, and it's not much longer now. That's what I took from these 15 weeks.
