What we know about psychiatric medicine has followed fairly conventional and predictable paths since the first selective serotonin reuptake inhibitors started to gain ground, it seems that psychiatry had entered a hiatus within neurological research. Looks like they were stuck on a path of no return. While the brain is in the middle of its most complex and detailed analysis in science, it seemed impossible for neurologists to understand other mechanisms that would solve such basic human afflictions, things that are written into our ancestral stress. It was then decided that everything boiled down to the logic of neurotransmitters, especially Serotonin, which became the main target of cutting-edge medications. Serotonin would explain and resolve everything. Selective reuptake inhibitors in turn would then be the final solution for any depression, anxiety, disorders, phobias, crises and so on.
SourceWhich of course didn't happen, and of course, since we understood that this was the way forward, we went deeper and deeper into the same theme. And little by little, medicines became more "filtered", "tuned" to hit the target supposedly responsible for the damage that was consuming us. At the beginning of the text I mentioned that this path narrowed down from selective reuptake inhibitors, but they weren't the first medications that helped us to heal from mental problems. Before that came the tricyclics and their immense action potential, much denser and more intense than refined selective inhibitors. Tricyclics did not only target serotonin (if this was the target), but because they were more rudimentary (or dirty, as they are called), they hit several neurotransmitters at the same time, such as noradrenaline, acetylcholine, dopamine, muscarinic and nicotinic receptors, among others. It is not by chance that many people who are not successful in the treatment with "filtered" selective inhibitors are much more successful with a tricyclic, because this probably shows that some other action of this drug made the difference and not the constant focus on serotonin , as preached by the generations that were born in the study of psychiatry in this Serotoninergic era.
SourceI'm not an expert to opine with such certainty about the error of this line of research that has narrowed itself more and more (until it reaches the limit of narrowing), but what I do know is that as a patient I can confirm that this hyper-focus on serotonin it didn't solve things for me any more than it did for a lot of people. Among these, it is worth mentioning the incredible advance that has been achieved in research with Ketamine, which until recently was seen solely as an anesthetic (human and animal) and also as a drug of abuse. And now we not only have countless evidences of its incredible therapeutic potential to treat depression resistant to conventional pharmacological treatments, but we also have this therapy already in effective operation in several countries, following a guided use protocol with application of the substance in the hospital itself (or specific location) with the presence of a medical professional. The patient usually receives a small weekly or biweekly dose directly into the vein, goes through an experience of one or two hours very similar to a psychedelic trip (since Ketamine is a powerful dissociative) and after its use, both the patient and his patients tests show that an improvement far superior to any antidepressant is noticed and lasts for at least 10 days. No need for daily doses or anything like that. What mechanisms bring these benefits and why they occur is still being studied. SourceLikewise, we also have the growing research and independent use of microdoses of psychedelics for the treatment of countless disorders, from severe depression to mild anxiety. The wave of research regarding microdoses has gained ground mainly with LSD fractions (lysergic acid) and also with Psilocybin, an active ingredient present in some mushrooms, such as Psilocybe Cubensis. Both are drugs that until recently had a bias almost solely focused on recreational use for the "great crowd", but in the counterculture and in many circles that study and research alternative plants and treatments, they were already seen as curative substances and with incredible treatment potential for countless psychological and emotional problems. However, still in a pattern of high doses and focused on an "intense trip". Something that does not exist in the concept of microdose. The idea of a small fraction of the substance previously used to achieve psychedelic intoxication makes us think that perhaps what until now was seen as an "acid trip" for example was actually nothing more than an "overdose of the substance" and that, like a medicine, if the acid is used correctly and in an exact dosage for the person, it can have benefits as advanced as some other medicines (and as we've seen, often better than many of these medicines).

Definiu-se então que tudo se resumia na lógica dos neurotransmissores, em especial a Serotonina, que tornou-se o alvo maior dos medicamentos de ponta. A serotonina explicaria e resolveria tudo. Os inibidores seletivos de recaptação por sua vez então seriam a solução final para qualquer depressão, ansiedade, transtornos, fobias, crises e assim por diante.
SourceO que naturalmente não aconteceu, e claro, dado que compreendíamos que era esse o caminho a se avançar, aprofundamos mais e mais dentro do mesmo tema. E pouco a pouco os medicamentos tornaram-se mais "filtrados", "afinados" para acertar o alvo supostamente culpado pelos estragos que nos consumiam. No começo do texto mencionei que esse caminho se afunilou à partir dos inibidores seletivos de recaptação, mas eles não foram os primeiros medicamentos que nos ajudavam a se curar de problemas mentais. Antes disso vieram os tricíclicos e seu imenso potencial de ação, muito mais denso e intenso que os refinados inibidores seletivos. Os tricilicos não atingiam apenas a serotonina (caso este fosse o alvo), mas por serem mais rudimentares (ou sujos, como são chamados) atingiam vários neurotransmissores ao mesmo tempo, como noradrenalina, acetilcolina, dopamina, receptores muscarinérgicos e nicotinicos, entre outros. Não por acaso muita gente que não tem sucesso no tratamento com os "filtrados" inibidores seletivos tem muito mais sucesso com um tricíclico, por que, isso provavelmente mostra que alguma outra ação desse medicamento é que fez a diferença e não o foco constante na serotonina, como pregam as gerações que nasceram no estudo da psiquiatria dessa era Serotoninérgica.
SourceEu não sou especialista para opinar com tanta certeza à respeito do erro dessa linha de pesquisa que foi se afunilando cada vez mais (até chegar no limite do afunilamento), mas o que sei é que como paciente posso confirmar que esse hiper-foco na serotonina não resolveu as coisas para mim assim como não resolveu para muita gente. E a prova disso inclusive é exatamente que, finalmente, depois de muito tempo temos visto um levante na área científica onde se tem dado visão e espaço para pesquisas de borda e que até um tempo atrás poderiam ser consideradas absurdas para tratar problemas mentais.
SourceEstou falando de mecanismos que ainda não compreendemos e que ainda são muito novos, mas que se mostram nitidamente promissores e muito mais sofisticados do que a redundante e maçante pesquisa nos neurotransmissores já supracitados. O paciente costuma receber uma pequena dose semanal ou quinzenal diretamente na veia, passa por uma experiência de uma ou duas horas muito semelhante a uma viagem psicodélica (já que a Ketamina é um dissociativo potente) e após o seu uso, tanto o paciente quanto seus exames comprovam que uma melhora muito superior a qualquer antidepressivo é notada e perdura por pelo menos 10 dias. Sem a necessidade de doses diárias nem nada do tipo. Ainda está sendo estudado quais mecanismos trazem esses benefícios e por que eles ocorrem. Inclusive esse é um tema que quero me aprofundar e quando eu compreender certamente trarei mais detalhes para vocês.
SourceDa mesma forma temos também a crescente pesquisa e o uso independente das microdoses de psicodélicos para o tratamento de inúmeros distúrbios, desde depressão severa até ansiedade leve. A onda de pesquisas à respeito das microdoses tem ganhado espaço principalmente com frações de LSD (ácido lisérgico) e também com Psilocibina, princípio ativo presente em alguns cogumelos, como por exemplo o Psilocybe Cubensis. Porém, ainda num padrão de doses altas e focadas em uma "intensa viagem". Coisa que não existe no conceito da microdose. A ideia de uma pequena fração da substância utilizada anteriormente para alcançar uma intoxicação psicodélica nos faz pensar que talvez o que até hoje era visto como uma "viagem de ácido" por exemplo na verdade não era nada mais do que uma "overdose da substância" e que, tal como um remédio, se o ácido for utilizado corretamente e numa dosagem exata para a pessoa, ela pode ter benefícios tão avançados como alguns outros medicamentos (e como temos visto, muitas vezes melhor que muitos desses medicamentos).
SourceExistem ainda outros vários caminhos que vem se abrindo dentro da psiquiatria, mas tudo ainda muito novo e inicial. Será necessário tempo para vermos reais mudanças nos tratamentos convencionais e em suas insuficientes qualidades de ação.
