And with this saying we begin to describe a long story of a patient who was well known in our operating room for "refracturing" the same site where he had previously had surgery for a fall. Although there are many cases of "refractures", resulting from mishandling or neglect of the patient, not many are the cases that take a long time until the time of a second surgery.

“En el mundo no hay nada difícil siempre que seamos perseverantes.”
Y con este refrán empezamos a describir una larga historia de un paciente que fue muy conocido en nuestro quirófano por “refracturarse” el mismo sitio donde anteriormente se había operado por una caída. Aunque existen muchos casos de “refracturas”, producto de un mal manejo o descuido del paciente, no muchos son los casos que transcurren mucho tiempo hasta el momento de una segunda intervención quirúrgica.
Case
Daniel Méndez, a 33 year-old computer engineering worker who was in a traffic accident in mid-May 2002, after riding a motorcycle at high speed recklessly and under the influence of alcohol, had a severe traumatism to his left forearm. That same year he underwent a procedure of "reduction and osteosynthesis" due to fractures at the forearm level, two metal plates were placed on him and after 4 months of rehabilitation he was finally able to return to work in a satisfactory manner.
On June 23, 2017, Daniel was working and helping in a construction site near his home, after lifting a heavy object he slides near the edge of a roof and falls sharply, so he is taken immediately to the Maracay Hospital emergency room and after being evaluated and x-rayed we noticed an unusual event....
Caso
Daniel Méndez, un trabajador de ingeniería informática de 33 años de edad acude a mediados del mes de mayo de 2002 por un accidente de tránsito, luego de ir en una motocicleta a alta velocidad de forma imprudente y bajo efectos del alcohol tuvo un traumatismo de fuerte intensidad en su antebrazo izquierdo, motivo por el cual es sometido ese mismo año a un procedimiento de: “reducción y osteosíntesis” por fracturas a nivel del antebrazo, le fueron colocadas dos placas metálicas y tras 4 meses de rehabilitación finalmente pudo reincorporarse al trabajo de manera satisfactoria.
El día 23 de Junio de 2017, Daniel se encontraba trabajando y ayudando en una construcción cerca de su casa, luego de levantar un objeto pesado él se resbala cerca del borde de un techo y cae aparatosamente, por lo que es llevado de inmediato a emergencias del Hospital de Maracay y luego de evaluarse y realizarse estudios radiográficos nos percatamos de un evento poco común...

Double Fracture
At that time we did not know about the first surgery he had undergone, but the X-ray said it all. Not only had the same surgical site been fractured, but the same two bones had been broken from the severe trauma and a distal ulnar radius dislocation had occurred. In other words, the wrist bones were separated by complete tearing of the ligaments.
It was definitely not a very easy scenario for Daniel, a situation that we made him understand from the very first moment, so we decided to hold a meeting with all the orthopedic surgeons at the Hospital to propose the best treatment. A quick and effective solution. Daniel is a young adult who needs to get his upper limb up and running as quickly as possible.
It was a week after stabilizing from the other minor wounds and healing that we decided to propose a complex but definitive method of surgery:
In short, the plan was to remove the previously installed metal plates and screws and replace them with new plates and screws. Why don't you leave the same ones and reposition them? Some will ask, as it turns out that even if they are not damaged, when they suffer a trauma of this intensity their resistance is lost, so they are very inefficient in stabilizing another fracture. It's a risk we didn't want to take because of our patient's high demands on physical work. On the other hand there was the big question of how to stabilize the dislocation of the wrist without prolonging the surgery for too long.... The solution, a wire that would go from one end to the other through the two bones of the wrist, of course, a strong surgical wire called: Kirshner's wire with a thickness of 2.0mm in diameter. Sort of like going through two pieces of wood with a screw to keep them in place. After this, wait for the body to do its part to eventually repair the site of the injury.
Doble Fractura
Para ese momento desconocíamos de la primera cirugía que le habían realizado, pero la radiografía lo decía todo. No solo se había fracturado el mismo sitio de la cirugía sino que los mismo dos huesos se habían se habían roto por el fuerte traumatismo y había ocurrido un luxación radio cubital distal. En otras palabras, los huesos de la muñeca estaban separados por rotura completa de sus ligamentos.
Definitivamente no era un panorama muy fácil para Daniel, situación que le hicimos entender desde el primer momento, así que entre todos los cirujanos ortopedistas del Hospital decidimos realizar una reunión para proponer el mejor tratamiento. Una solución rápida y efectiva. Daniel es un adulto joven que necesita realizar y poner en funcionamiento su miembro superior lo antes posible.
Fue una semana después luego de estabilizarse de las otras heridas menores y cicatrizar que decidimos proponer un método de cirugía compleja pero definitiva:
En resumen, el plan era retirar las placas y tornillos metálicos antes colocados y sustituirlos por otras placas y tornillos nuevos. ¿Por qué no se dejan las mismas y se reposicionan? Preguntaran algunos, pues resulta que aunque no se vean dañados, al sufrir un traumatismo de esa intensidad su resistencia se pierde, por lo que resultan muy ineficientes para estabilizar otra fractura. Es un riesgo que no quisimos correr por las altas demandas de trabajo físico de nuestro paciente. Por otro lado estaba la gran duda de cómo estabilizar la luxación de la muñeca sin prolongar mucho tiempo la cirugía… La solución, un alambre que atravesara de un extremo a otro los dos huesos de la muñeca, por supuesto, un alambre quirúrgico resistente llamado: Alambre de Kirshner con un grosor de 2.0mm de diámetro. Algo así como atravesar dos pedazos de madera con un tornillo para que se mantengan en su sitio. Luego de esto, esperar a que el cuerpo haga lo suyo de reparar con el tiempo el sitio de la lesión.
The Surgery
The day of the surgery wasn't exactly as early as we wanted it to be, largely because we waited a long time for Daniel to get the necessary supplies for his surgery, since insurance doesn't cover the cost of these materials because they are so expensive, patients have to pay for the materials themselves.
Once we had everything at hand, it was time to start the work. So we all went into surgery. I remember that day he asked me, "Will everything be all right, Doctor?" And with a firm and sincere voice I replied: "Don't worry, Daniel, we won't get out of there without giving our best effort."
Once under general anesthesia, the site of the injury was cleaned up and we were with a large team headed by master hand surgeon Dr. Juan Carlos Castillo, a renowned surgeon, long experience in Venezuela, professor and great friend.
The incision was made through the anterior scars in order to avoid unsightly injuries. Once we opened the way between muscles and tendons we noticed a new bone tissue magma "de novo", which even if it was completely benign we had to remove momentarily to remove remains that could affect proper healing.
It was an hour before we removed all the screws and plates from the old surgery, cleaned and guided with an X-ray machine in the operating room and were able to locate the new position of the new plates, it was not easy, since the bone had suffered a previous fracture and the removal of the old plates made the area even more fragile. Once satisfied with the results it was time to apply force to our drill to hit the wire between the two bones. If you look at your final x-ray you will notice that it is located just at the level of your wrist, achieving its perfect alignment.
Three and a half hours of hard working surgery and I was finally able to sit down while my assistants sutured the skin, to one side the master Dr. Juan Carlos Castillo, "Everything went as we expected" and with a smile of relief I was able to have a coffee.
After 7 months of physical therapy, monitoring and removal of the wires, Daniel has long been at his new job in the office and away from any threat, avoiding heavy objects. I don't forget that handmade strawberry yogurt that his mother brought as an act of gratitude, it was very tasty.... For it had the aroma of all thanks.
La Cirugía
El día de la cirugía no fue exactamente tan temprano como quisimos que fuera, en gran parte debido a que esperamos por mucho tiempo que Daniel consiguiera los implementos necesarios para su cirugía, puesto que el seguro no cubre el gasto de estos materiales por lo costosos que son, los pacientes tienen que costear por si solos los materiales.
Una vez tuvimos todo a la mano, fue tiempo de iniciar el trabajo. Así que nos avocamos todos al quirófano. Recuerdo que ese día él me preguntó: “¿Todo saldrá bien Doctor?” Y con una voz firme y sincera le respondí: “Tranquilo Daniel, no saldremos de ahí sin dar nuestro mejor esfuerzo.”
Una vez que estuviese bajo anestesia general, se realizó limpieza del sitio de la lesión y estuvimos con un gran equipo encabezado por el maestro cirujano de mano Dr. Juan Carlos Castillo, un cirujano de reconocido renombre, larga trayectoria en Venezuela, profesor y gran amigo.
La incisión se realizó a través de las cicatrices anteriores con el fin de evitar lesiones antiestéticas. Una vez que se abrió pasos entre músculos y tendones nos dimos cuenta de un magma de tejido óseo nuevo “de novo”, que aunque fuese totalmente benigno tuvimos que removerlo momentáneamente para eliminar restos que pudiesen afectar una adecuada cicatrización.
Pasó una hora hasta que retiramos todos los tornillos y placas de la antigua cirugía, limpiamos y guiados con un equipo de radiografia en quirófano pudimos ubicar la nueva posición de las placas nuevas, no fue fácil, en vista de que el hueso había sufrido una fractura anterior y el retirar las antiguas placas hacia aún más frágil la zona. Una vez satisfechos con los resultados era hora de aplicar fuerza en nuestro perforador para acertar el alambre entre los dos huesos. Si observan su radiografía final se darán cuenta de que se ubica justo a nivel de la muñeca logrando su alineación perfecta.
3 horas y media de una cirugía de tanto trabajo y finalmente pude sentarme mientras mis ayudantes suturaban la piel, a un lado el maestro Dr. Juan Carlos Castillo, “Todo salió como esperábamos” y con una sonrisa de alivio pude tomar un café.
Luego de 7 meses de fisioterapia, de control y retiro de los alambres, Daniel se encuentra desde hace mucho en su nuevo trabajo en la oficina y lejos de cualquier amenaza, evitando objetos pesados. No se me olvida aquel yogurt de fresa artesanal que trajo su madre como acto de agradecimiento, era muy sabroso… Pues tenía el aroma de todos los agradecimientos.
Final Result: Successful alignment of the fractured bone with a double plate system and placement of a surgical wire in the wrist. // // Resultado Final: Alineación exitosa del hueso fracturado con doble sistema de placas y colocación de un alambre quirúrgico en la muñeca.
